Keith,
With a small change in the design, you can do it all on the lathe. Just skip the end mill work. You will have to drop the screw in the hole rather than slide it in from the side but the rest is the same. This change is a good way to go when you get to screws smaller than 6-32 because it is hard to find end mills smaller than about 0.1" in diameter.

The vise clamps a #6 or #8 screw such that it's end can be easily and safely
beveled on a grinding wheel. The vise is similar to a pin vise except that
